BASIC REQUIREMENTS:
1. Applicant must be at least 21 years old with a minimum fixed monthly income of P10,000 for BPI Express Credit Classic, BPI Blue MasterCard, BPI WorldPerks MasterCard, Petron-BPI MasterCard, BPI Edge MasterCard and P50,000 for BPI Gold MasterCard (excluding overtime, commissions or service fees.)
2. You must have a business or residence contact number.
3. If you are non-Filipino, you must be a permanent resident of the Philippines (please include a photocopy of the Alien Certificate of Registration, work permit or Embassy Accreditation Papers.)
4. You must completely fill-out the online application form.
5. To complete and process your online application, please FAX the required documents to 845-5738 Routing Code 20022.
6. Your application will be canceled if documents are not received within 3 banking days.

DOCUMENTS REQUIRED:
Only applications with the following complete documents will be processed.

1. Proof of Identification
Applicant must FAX photocopy of at least two (2) government-issued photobearing ID's:
a. Driver's License
b. Passport
c. SSS / TIN / GSIS ID
d. Philippine Regulatory Commission (PRC) ID
e. Postal ID
f. Others

A BPI WorldPerks MasterCard applicant who is an existing WorldPerks member may FAX ANY of the following document(s):
i. Photocopy of WorldPerks ID
ii. Photocopy of the latest E-mileage summary
iii. Photocopy of any WorlPerks membership document that indicates the valid and registered
WorlPerks Name and the number in the WorldPerks membership program

2. Proof of Income
If applicant is employed, please FAX a photocopy of the following:
a. Latest Income Tax Return (ITR) with BIR or bank stamp; OR
b. W2 (BIR Form 2316)
If applicant is self-employed, a proprietor or a businessman, please FAX a photocopy of the following:
a. Most recent Audited Financial Statements (AFS) with BIR or bank stamp; AND
b. Most recent Income Tax Return (ITR) with BIR or bank stamp

In addition to the above requirements, you may fax a photocopy of the following documents, as these may facilitate the processing of your application:
1. Certificate of Employment
2. Previous month's pay slips
3. Previous month's Credit Card Statement/s (You must be an existing principal cardholder for at least one year with the card/s in active current status.)
